The Captain's Information Sheet has lots of information that will help you be a successful captain. Please take a few minutes to review this sheet and if you have any questions, please give your Cup Coordinator a call.
If you need a spare Line- Up or Revised Line-up Form, these are also available.
If you need an extra Score Sheet, you can click on the button below. Remember to send one copy to the Match Chairman (Janet Dzubow) and one copy to your Cup Coordinator.
